HIV Care Case Management

HCH case managers can assist in enrollment and navigation of federal, state, and local assistance programs. These financial assistance programs help cover the cost of medication and treatment that regular insurance might not cover.

Hepatitis C Care Case Management

Assistance in enrollment of GlaxoSmithKline and other assistance programs to help cover the cost of medication.

HIV Treatment and Care

We offer primary care, behavioral health, care coordination, and medical case management for people living with HIV/AIDS (PLWH).

PrEP | PEP Services

PrEP (pre-exposure prophylaxis), is a daily medication that HIV-negative individuals can take as an extra layer of protection against HIV infection.

PEP (post-exposure prophylaxis), is an emergency medication that HIV-negative individuals can take in case of accidental exposure to HIV – such as a condom breaking or tearing. PEP must be taken within 72 hours (3 days) after a possible exposure to HIV. PEP can be accessed from any emergency room.

PrEP NAVIGATION

PrEP is covered by most insurance; however, there are programs for those who do not have insurance. Our PrEP Case Managers are able to assist with enrollment and care coordination.

FREE confidential rapid testing, regardless of insurance or legal status. Walk-in and same-day appointments are available during regular office hours: 9 AM – 5 PM.
